Health care services offered at home, at school, and in the community, as well as access to supported living programs and long-term care, are coordinated by Local Health Integration networks (LHINs) to ensure people of all ages can make informed choices about their care, when and where they need it.
Professional members of the diabetic foot care team can be accessed through the LHIN Home and Community Care. Integrated team members include Physiotherapists, Occupational Therapists, Social Workers, Registered Dietitians, Nurses, Enterostomal Therapists (ETs) and Personal Support Workers. For individuals at risk, it is recommended that their team include members who specialize in diabetes and diabetic foot management.
