The SWRWCP is now providing VIRTUAL wound management guidance and support to health care providers within the following Ontario counties: Grey/Bruce, Huron, Perth, London/Middlesex, Elgin, and Oxford.
This service can be utilized to help support providers who are caring for individuals with or at risk for wounds. Secure technology will be used to support this service to ensure patient privacy (e.g. fax, phone, OTN).
If you and your patient/Substitute Decision Maker are interested in a Virtual Wound Care Consultation, please complete the referral form Click Here and fax to 1-855-223-2847. Response time time for consult requests is within 5 business days. Urgent patient concerns should be directed to the most responsible health care provider. *The referring health care provider is required to obtain Patient/Substitute Decision Maker consent prior to referral submission.
